An elderly man has died after a horror head-on crash in Welshpool that saw five others rushed to hospital.

The 88-year-old was driving a silver Hyundai Elantra north-easterly on Leach Highway at about 2.45pm on Tuesday when his car veered over the median strip and into oncoming traffic, ploughing into a Nissan Patrol, Toyota Corolla and a Honda HRV.

The elderly driver died at the scene while two motorists, a man and a woman, were taken by St John Ambulance under priority one conditions to Royal Perth Hospital.

One is in a serious but stable condition the other is in a stable condition.

A further two people were taken to RPH while a fifth person was taken to Fiona Stanley Hospital, all under normal road conditions.

The crash caused traffic chaos, with the highway closed westbound on the Orrong Road overpass for five hours as emergency services worked to clear the scene.

Traffic was banked up all the way back to Tonkin Highway.
